The realities of working within a failing and bureaucratic system are explored in this collection of stories from two GPs. They argue that the gap left by the old-fashioned GP has been filled by protocol, which influences professional relationships between and within organizations and with patients/clients/service users. They suggest that the frustration of being unable to connect with another human being who will listen to their particular question and give them something other than a generic answer has changed the way in which we work as GPs and the care we give.

The following is the rephrased text:

Working within a failing and highly bureaucratic system is a challenging task for GPs. This collection of stories from two practising GPs explores the reality of balancing regulation, relationships, autonomy, and standard practice while navigating the complexities of algorithm versus individual attention.

While we recognize the importance of being bureaucrats and being embedded within and accountable to the systems we are in, we also want to consider how the gap left by the traditional GP has been filled. We use personal anecdotes to shed light on the impact of bureaucracy on our ability to maintain a nuanced, individualized approach to each patient and encounter. We question the prominence and effectiveness of protocol and explore how it influences professional relationships, both within and between organizations, and most importantly, with patients/clients/service users.

In today's world, we have become accustomed to automated telephone lines, chatbots, and website FAQs, often feeling frustrated when we cannot connect with another human being who can listen to our specific question and provide us with a personalized answer. The same issues that society faces at large have also impacted the way GPs work and the care we provide.

The challenges faced by GPs in this complex system are numerous. Bureaucracy can lead to a lack of flexibility and responsiveness, making it difficult to adapt to changing patient needs and circumstances. Protocols and guidelines can restrict the ability to deviate from standard practices, even when it is in the best interest of the patient. At the same time, the emphasis on efficiency and productivity can lead to a rush to judgment and a disregard for the holistic nature of healthcare.

However, there are also opportunities for GPs to navigate these challenges and provide high-quality care within the constraints of the system. By building strong relationships with patients and collaborating with other healthcare professionals, GPs can work to personalize care and ensure that patients receive the best possible treatment. They can also advocate for changes within the system to improve the working conditions and support for GPs, such as reducing administrative burdens and increasing funding for patient care.

In conclusion, working as a GP within a failing and highly bureaucratic system is a complex and challenging task. However, by recognizing the impact of bureaucracy and seeking opportunities to navigate it, GPs can continue to provide high-quality care and make a positive impact on the lives of their patients.
