Since the 1970s, the International Atomic Energy Agency (IAEA) has been actively engaged in the analysis of fuel failures in water-cooled reactors under normal (non-accident) operational conditions. This revised edition of IAEA Nuclear Energy Series No. NF-T-2.1 offers comprehensive information on all aspects of fuel failures in contemporary nuclear power plant operations. It summarizes the occurrences, mechanisms, and root causes of fuel failures, as well as the prevention and management strategies employed in plant operations for 97% of light and heavy water-cooled nuclear power units worldwide from 2006 to 2015. The present publication includes data on fuel failures from 1987 to 2006, extracted from three previous IAEA fuel failure reports, and analyzes them alongside the 2006–2015 fuel failure data to unveil long-term trends in fuel performance. In addition to fuel rod leaks, the report also addresses fuel structural damages and other fuel assembly-related issues.
